KHA is collaborating with hospitals and community partners in a shared effort to address resources and community support for various needs that our community residents have every day. Through these partnerships, the community coalitions will build stronger, healthier communities by uniting the key local players to have conversations around issues such as SDoH, opioid use and misuse, chronic disease self-management, care coordination, immunizations, emergency preparedness, and so much more.
Community Coalition partners include but are not limited to patients/families, clinician practices, nursing homes, hospitals, home health, hospice agencies, pharmacies, area agencies on aging, community health centers, fire departments, EMS providers, patient advocacy organizations, research institutions and universities, state and local agencies, medical /professional associations, faith-based organizations, community organizations active in disasters, senior housing, local AARP chapters, and substance use recovery organizations. If you or your organization make an impact on a community member’s life, we want to invite you to the coalition meetings.
