KHA Quality Webinar: Peer-to-Peer Learning on the Importance of Compassionate Community Projects

Ephraim McDowell Regional Medical Center and Murray-Calloway County Hospital showcased what it means to evaluate the needs of their communities, self-reflect on the current processes in place, and bring a multidisciplinary team together to improve the care pathway for chronic heart disease and breast cancer patients.

Rachel Holton, Cardiovascular Quality and Accreditation Manager for Ephraim McDowell Regional Medical Center, will showcase the process of standardizing an evidence-based heart failure care model spanning inpatient care, outpatient clinic services, and transitions of care to reduce variations, improve care coordination, optimize guideline-directed medical therapy, and strengthening patient education to deliver high quality heart failure care across the continuum.

Jaclin Parker, Nurse Navigator for Murray-Calloway County Hospital, will showcase the implementation of a systemwide design of the breast diagnostic and oncology referral process to create a rapid, reliable, multidisciplinary pathway spanning multiple departments across the organization.
