What is a Community Coalition?

A coalition is an alliance of individuals and/or organizations working together to achieve a common purpose. Each coalition has a common goal which helps to establish and build relationships within a community where better ideas and outcomes can be achieved together.

Who should be a part of the Community Coalition?

  • Patients and/or Families
  • Clinicians practices (Physician, nurses, etc.)
  • Nursing Homes
  • Hospitals
  • Home Health
  • Hospice Agencies
  • Pharmacies
  • Area Agencies on Aging (AAA)
  • Community Health Centers
  • Fire Departments or EMS Providers
  • Patient Advocacy Organizations
  • Research Institutions and Universities
  • State and Local Agencies
  • Medical/Professional Associations (AMA, AHA, APA, etc.)
  • Faith Based Organizations
  • Community Organizations Active in Disasters
  • Senior Housing
  • Local AARP Chapters
  • Substance Abuse Recovery Organizations
The partnership aims to support local community efforts to improve on the following goals:
  • Opioid Utilization and Misuse
  • Chronic Disease Self-Management
  • Care Coordination (Readmissions)
  • COVID-19/Immunizations
  • Emergency Preparedness.
